Resumo

Sickle cell disease is a very common genetic and hereditary disease in Brazil and around the world. It mainly affects the black population (black and mixed race). It is a serious disease due to the numerous complications and the development of aftereffects throughout the lives of those affected by it, but it is treatable. Racism was a major factor in the invisibility of the issue, with the consequent lack of health measures aimed at this condition, even after more than a century since its discovery. Due to the poor prognosis, the violation of the right to health and the racism that exists in the country, it is still a reality for black women with sickle cell disease to be encouraged not to have children, to have abortions or even to be sterilized. In the second half of the 20th century, after an intense struggle by the black movement for an effective health system, the creation of a specific policy aimed at the treatment of sickle cell disease was observed at the beginning of the 21st century. With advances in care and the advent of the drug hydroxyurea, it was possible to improve the clinical picture, increasing the life expectancy and quality of life of black women affected by this disease. Since the State of Rio de Janeiro is a leading player in the debate and institutionalization of public policy in the country and has the second highest incidence of cases of the disease, this study aimed to analyze, based on the narratives of black women with sickle cell disease, how the reproductive rights of these women are (or are not) met by the State Policy for Comprehensive Health Care for People with Sickle Cell Disease in Rio de Janeiro. This is a qualitative research study that used semistructured videotaped interviews, document analysis, and the application of a sociodemographic questionnaire. Five women diagnosed with sickle cell disease, who self-identified as black, aged between 35 and 65 years old, and with experience in the pregnancy process were selected. The narratives of the participants in this study, in summary, revealed the lack of guarantees for the exercise of the right to procreation by women with sickle cell disease; they denounced physical and psychological scars, the result of neglected health care provided to these women and the way in which health professionals understand their reproductive rights; they highlighted the need to create strategies to eradicate and/or minimize the impact of social ills and racial discrimination on the health of black women with sickle cell disease and pointed to the challenge of creating a way for these women to be truly heard. The research thus shows that the Comprehensive Health Care Policy for People with Sickle Cell Disease in the State of Rio de Janeiro still does not meet, in a sufficiently adequate and dignified manner, the reproductive rights of black women affected by this disease.
Acomete majoritariamente a população negra (pretos e pardos).
